Our physicians AND our staff assist you with the completion and uploading ALL of your documents to the OMMA for your license, including your required passport style photograph. Your application will be FULLY COMPLETE when you leave our offices. Please bring a photo ID and list of any medications you might be taking, if any.

Military Veteran – MMJ Recommendation
Special offer for our military veterans with valid military ID or other military service documentation. We provide your medical marijuana patient recommendation from our physician AND our staff assists you with the completion and uploading ALL of your documents to the OMMA for your license, including your required passport style photograph. Your application will be FULLY COMPLETE when you leave our offices. Please bring a photo ID and list of any medications you might be taking, if any.

Before or After Hours Private Consultation
Our physician and one assistant will meet with you one-on-one before or after normal business hours for a private consultation and MMJ recommendation. This service is designed for individuals who seek an extremely discrete physician visit and is by advance appointment only.

Note: The physician’s recommendation fees do not include the Oklahoma Medical Marijuana Authority (OMMA) fees due to the State for your two-year license. These fees are $104.50 unless you are on Medicare, Medicaid or SoonerCare then then it is $22.50 (please bring your card with you to receive this discount on your state fees).
